I think most embarrassing would have to happen in multiplayer games, as there would have to be somebody to see it
So here's mine.
I was playing MW2 recently, and snuck up behind a sniper. I couldn't decide how to kill him, thought that I should semtex his head or something. Before I could do anything, he got up and killed me. I was speechless
